New Patient Registrationregister_now

If you live within our practice area - which you can check using the tool below - you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill. You will also need to provide proof of identity.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are required to attend a New Patient Healthcheck with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Please note: if you are completely housebound and unable to attend the surgery, please inform the receptionist, who will make alternative arrangements for you.

We take your care very seriously, so failure to book or attend the New Patient Healthcheck will mean we will not be able to continue with your application to join the practice.

Medical treatment is available from the date of registration. Please contact reception for further information.

Out of Area Registrations

Unfortunately, we are unable to accept applications for patients residing outside of our practice boundary. Existing patients moving outside of our practice area will be required to re-register with another practice that covers their new address.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
