New Patient Registration
If you live within our practice area - which you can check using the tool below - you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You will also need to provide proof of identity.
You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Please call in to the surgery to collect the forms, or email us on ingol.broadway@nhs.net with your address.
The Practice recommends that all patients should attend for a New Patient Medical. If you are not on any medication and do not wish to have a New Patient Medical you are given the option to opt out. Please bring a list of your medication and a urine sample to your appointment should you wish to attend. Those patients on medication are advised to attend for a New Patient Medical.
Please note: if you are completely housebound and unable to attend the surgery, please inform the receptionist, who will make alternative arrangements for you.
We take your care very seriously, so failure to book or attend the New Patient Healthcheck will mean we will not be able to continue with your application to join the practice.
Medical treatment is available from the date of registration. Please contact reception for further information.
***New patients please note that the majority of our services, including all same day appointments, are provided from our Ingol Health Centre site***
Out of Area Registrations
Unfortunately, we are unable to accept applications for patients residing outside of our practice boundary. Existing patients moving outside of our practice area will be required to re-register with another practice that covers their new address.
